2. identity card (copied)
3. identity number of the region tax payer (copied)
4. SITU (copied)
5. UKL, UPL and AMDAL Document
6. The license of nuisance law
7. Photo 3 X 4 3 pieces

14 ( foorteen) working day

COST
Industry license :
1. Big Industry ( Investation Above 1 Billion Rupiah ) Costed Rp 500.000,- ( Five Hundred Thousand ) Added Legal Size Fee Rp. 100.000,-
2. Medium Industry ( Investation Rp 300.000 Till 1 Billion ) Costed Rp 376.000 ( Three Hundred Seventy Six Thousand) + Legal Size Fee Rp. 75.000
3. Small Industry ( Investatiopn Les Than 300.000.000 Costed Rp. 250.000 + Legal Size Rp. 5000

VALID ISSUE
5 Years, started with issued and industry list register is valid during the industry is still running

THE KEEPR’S OBLIGATION
1. The keeper must give /report periodically every 6 (six) month about activities and product of it’s production
2. He must not do activities out side off determined license that given

SANCTION
1. Sentenced 6 month in jail or fined as much as Rp. 5.000.000 ( Five million rupiahs )
2. Warning in written
3. Frozen the license
4. Drawing out the license
